Cartoon monkey writing face pageIf you're using research animals, you'll need to answer the question "Vertebrate animals, yes or no" in Item 5 on the face page of the PHS 398 (for paper applications) or in Item 2 of the Other Project Information component in your Grant Application Package (for electronic applications).

Remember that your application covers all performance sites, including subaward partners, collaborators, and others involved in the research. Even if the animal work will be done somewhere other than your institution, mark "yes."

Item 5a asks you to provide your IACUC approval date. Fill in the date your IACUC approved your protocol; a review is required at least every three years. If you plan to use just-in-time IACUC approval, write "pending."

If a performance site is doing the animal work for you, provide its IACUC approval date only if the site is assured by OLAW or "pending" if the site will use just-in-time approval.

Item 5b asks for your institution's animal welfare assurance number. Be sure to provide the assurance number of your institution (the one applying for funding) even if a performance site is conducting the animal work. If your institution doesn't have an assurance, enter "none."

To see if your institution or performance site is assured, check Domestic Institutions with a PHS Approved Animal Welfare Assurance or Foreign Institutions with a PHS Approved Animal Welfare Assurance.
