Predoctoral fellows and trainees do not incur a payback obligation. Postdoctoral fellows and trainees incur a payback obligation for the first 12 months of NRSA support, which may be paid back with an additional 12 months of NRSA post-doctoral support. See Fellowship Grants (F).
Yes. Except for the NIH Pathway to Independence Award (Parent K99/R00), you must be a U.S. citizen, non-citizen national, or have permanent residency status (an Alien Registration Receipt Card, Form I-551) at time of award or appointment. People on temporary or student visas are not eligible.

No. Training grants are awarded to eligible institutions that pay stipends and benefits to candidates selected for pre- and postdoctoral research training by the training grant program director and a selection committee.
