No, NIH does not anonymize applications, i.e., remove the identity of applicant institutions and investigators before peer review.

NIH acknowledges that concerns about bias, unconscious or otherwise, exist. NIH has previously blinded peer review, either as part of a pilot program, internal study, or novel funding initiative, but it is not normal practice. See Integrity and Fairness in Review for more on this topic.
